Complete the Reactivation Form to the right. We encourage you to bring the completed form to the next chapter meeting.  Please include the following information:

- Current name

- Address

- Chapter of initiation and date

- Last chapter with which you were active

- All previous names used

- Financial card number (if available) 

2. What are membership dues for Mu Kappa Omega Chapter?

Dues vary depending on your current membership status. To find out your exact payment, please email mkomembership@mukappaomega.com
